Retirement Plaques and Bonus request:

Are you retiring soon? If so, please complete the form below so we can help celebrate your retirement!

The information provided will help us determine whether you would like to receive a retirement plaque and whether you would like SPA representatives to attend your retirement celebration.

Please make sure your retirement date is accurate, as your SPA account will be deactivated effective that date. If you qualify for a Lifetime Membership, your membership type will be updated accordingly.

Lifetime Membership:

Upon retirement, an SPA member who has paid membership dues for at least their last 10 consecutive years will qualify for Lifetime Membership.

Lifetime Members will:

  • No longer be required to pay membership dues.
  • Be eligible to attend certain SPA events at no cost.
  • Receive complimentary admission to eligible events for the Lifetime Member and ONE guest only.
